Healthcare Assistance: Get help with health insurance enrollment, update your current health insurance information, and learn about important changes to Medicaid and your coverage.
Rent & Utility Assistance: Pre-register for an appointment to learn about the Catholic Charities Hawai’i Relief Program

REGISTRATION HIGHLY RECOMMENDED
Walk-ins are welcome, but we highly encourage you to pre-register by scanning the QR code so we know which services you are interested in receiving.

Please bring the following items with you

• Valid Photo Identification for ALL adults – front and back 
• Birth Certificate for at least ONE child under the age of 18 
• Proof of U.S. Citizenship or Valid Immigration Status for at least ONE household member 
• Proof of Residency for ALL household members – Valid Hawaii state ID, student ID, utility bill, rental agreement, etc. 
• Document of Extenuating Circumstances 
• Income statements for ALL members of household receiving income – One paystub within pay period dated from month prior to application date or month of application date. 
• Social Security, SSI, TDI, pension, disability, welfare/TANF deposit, unemployment, etc. 
• Valid Rental Agreement or current Mortgage Statement (please fill out attached third party form if requesting mortgage) 
• If you have a rent adjustment such as Section 8, please provide the current letter 
• Utility Bills for each utility request and Utility Consent Form filled out (if applicable) 
• Automobile loan statement, bill of sale, etc. (for automobile payments)
