Release Packet

The parent or legal guardian will need to provide the release packet that was received upon their child's release from the previous school.

  •  If the student is transferring from another Hawaii Department of Education School, documents may include:
    • A COPY of the latest Report Card
    • Student Health Record
    • Certificate of Release
  • If the student is transferring from a private school, another state or foreign country, documents may include:
    • A COPY of the latest Report Card
    • Student Health Record
    • Certificate of Release
  • If you have NONE of the above, the following information must be provided of the previous school:
    • School Name;
    • Address;
    • Phone number;
    • Fax number; and
    • A Certificate of Release 
  •  If the student will be enrolling in Kindergarten for the first time and ff applicable,  please provide a verification of attendance from the preschool attended. (The name of the school with the start and end dates of the child's attendance.)
