Completed forms and original documents are required at time of appointment.
*We DO NOT accept incomplete packets*
Required Documents:
- Completed Registration Form
- Birth Certificate/Passport
- Parent/Guardian ID
- Proof of Residence - utility bill, rental agreement, notarized letter
- Health Records - Form 14 - current TB, Varicella clearance, required immunizations, school physical (need doctor note with appointment date/time if not completed by school start date)
Your child(ren) has to be 5 yrs. of age by or before July 31, 2026 to attend Kindergarten for the 26-27 SY.
